Hi, welcome to Quoox University.
So in this course of videos, we’re looking at forms and questionnaires, and now I want to show you how you create a member form.
So this is a bit different to creating a form or questionnaire under Connect and Promote. So if you haven’t already watched those videos, go check it out, but for creating a member form, we’re gonna go into the main menu and click Settings.
And then we’re gonna go into Member Forms, just about halfway down.
Now, what you’ll see here is we have some that are already uploaded. So a Par-Q form, maybe there’s an onboarding interview questionnaire that you’d like filled out, and functional movement screen information. So, as always, under actions, you can click on the pencil and edit these forms.
And what you’ll see is that you have the name, a description, what the document type is, and any questions that you’ve been adding here.
To create a new one from the member form listing, click on the Add button. It will ask you for a form name. So let’s just say Test, Testing Form, and what document type it is. It’s gonna be a member form. Then we can add our questions.
So maybe it’s, “What are your fitness goals?” Then you can add either a single or multi-line response. It might be multi-line. And you can make it required or unrequired by ticking that box.
We click on the tick to save it and then click Save and we’ll have created our new member form, okay.
