Hi, welcome to Quoox University.
In this video, I’m going to show you how you can set up a form and questionnaire.
So we’re going to start by going to our main dashboard at my.quoox.com We’re going to log in with our business information and we’re brought to our main dashboard.
Chapter 1: Finding Forms & Questionnaires
Now for Forms and Questionnaires, we’re going to go straight over to our main menu on the left-hand side, we’re going to click into collect, Connect and Promote, (sorry!).
And then we’re going to go to Forms and Questionnaires.
Now what you’ll see here, because it’s a demo account is there’s already a few forms created and there’s different pieces of information that you can see straight from the forms listings, if you will.
Chapter 2: Overview & Actions
So you have the form name, how many responses you’ve had for said form.
When your first response was and your last response.
And as always, actions that you can take straight from the main page under the Actions tab here as well.
So you’ve got Edit, Delete, Ad hoc send out as well.
You can clone it, schedule it, or view and edit a schedule you’ve already created, download all your responses or promote by the marketplace should you wish.
So we’re going to focus in this video on how you add the form. Now we know where to find it!
Chapter 3: Adding A Form
So you click on the green button Add Form, and it will come up with some specific questions and or, or pieces of information that you need to fill out.
So what is the name? It might be your “New Starter Welcome Form.
“New starter form for all, to fill out with health information”, for example.
Just a good description as to what exactly that form is.
Chapter 4: Settings
Now you have a few options within this page already.
So you can either raise an alert when a member completes the formal questionnaire, you can tick or untick this.
You can make the form valid for a set number of days. So if you need it completed within the first week, maybe you say you need to do it within seven days. Otherwise it will be like removed from your account and you won’t be able to fill it in and we’ll have to manually add it. Again that’s another way you can keep track of people that are completing their forms.
You can add different highlight colours as always.
You can add specific images. So maybe you’ve got a screenshot or a PDF version of the form that you want to upload to use as an image.
And then you can also set a default allocation. So what this basically means is that you’d automatically assign the form for any new member to fill out once that member’s being created. Now bear in mind, if you don’t want specific memberships or trial memberships, or maybe you’ve got a page you got members to fill out a certain form, I wouldn’t tick this box. However, if you’re happy for everybody to have it, maybe it’s a PAR-Q or a waiver or anything like that, then you can tick yes to make it mandatory for every new member to fill out and you can make it mandatory after one day for example.
All right. So we’re going to click Save.
Once we’ve clicked Save, what you’ll see is we’ve been given two new tabs. So we’ve been given the option to configure our questionnaire or our form, and we’ve got the option to review our responses as well.
Chapter 5: Configuring Your Form
So to configure your form, you can add a question.
You can add any question you like here, so it could just be their name.
And then you can put in what type of answer they can give.
Now there’s lots of options here. You have everything from:
- the date and time, to
- a drop down box, to
- their email address, to
- a phone number, or
- yes/no answers for example
For this, you might want to put Long Text, you don’t know how long someone’s name is!
And again, what you’ll also see is a tick box that selects that the option, the question can’t be skipped.
So you tick that, it means they have to fill out their name. It might be that you want to get any health concerns for example. Again, you could use Long Text for that. Um, it could be multiple choice. Maybe you’ve got a list of health check boxes that you want them to fill out.
It’s completely up to you how you configure this. So you can, you can work on this as, as much, or as little as you like in terms of how much you beef it out, but it’s up to you, how you use it, but you can configure it from within this page.
Chapter 6: Viewing Responses
And then as and when you get responses, they’ll come through to the third and final tab.
So that’s how you can create your form or questionnaire.
