Hi, welcome to Quoox University.
In this course, we’re looking at forms and questionnaires.
So if you’ve watched my previous video on how to create one, stick with me and we’ll look at how we configure and set some questions and answers. If you haven’t, I suggest you go back so you know where to start!
So we’ll have created a form.
We’ve done a test for example. So let’s go back into our test form by clicking the pencil.
Now what you’ll see on the first page, which is great is that there’s a possible configuration issue with this form. So the system’s telling you if something won’t be working straight away.
So we’re gonna go back into Configuration and we’re gonna see what that might be.
So we’ve got a question: Number three. “How are you feeling right now?”
That’s a multi choice multi select question. That’s where our error was.
So we’re gonna click on that and it’s now opened up the option to add answers for someone to select.
So we can add an answer that says, “Great”.
You could add an icon if you wanted to so that could be a nice, big, happy smiley face and maybe very green.
And then you can click the tick under Actions to add that answer.
Then you could put another one that says, “Not great”.
Make that red if you wanted to.
Put a sad face. And then you have other options underneath where this can either raise an alert or trigger free text.
So if we trigger free text, what that means is when they select “Not great”, they could give us a reason as to why.
So you could put in the text box here, “give reason below”, for example.
Then you could also select… It raises an alert for you to check in on this member, if they’re not feeling great right now.
So you could select the tick under Actions and you can now see that this answer will raise an alert for you and trigger a free text box for them to give their reasons below.
You can then save the answers.
And now for your multi choice question and multi select answers, you’ve got the answers that are optional for your member.
If you want to add another question, you click the green button on the left-hand side.
You can enter your desired questions. So maybe it’s “How many times have you worked out this week?” for example.
And again, you have all of the same dropdown options to make the answer as you want it to be entered.
So maybe it’s a decimal number. Maybe it’s a dropdown from one to seven or one to 10, however many times you suggest someone works out a week, for example, or it could just be free text and you allow them to put some information in.
So I’m gonna select a decimal number and you can also select whether the question can be skipped or not.
So essentially what that means is if we tick this button, the member won’t be able to move through to the next question until they’ve answered that one.
All right. So that is how you can start configuring your questions and answers when you select multi choice answer options.
